• Aging population 60 years and older
• Disabled population 18-59 years old
• Handicapped population 18 years and older
• People who have impairment which would prohibit them from living independently without supportive services and are at least 18 years of age.

The process for enrollment:

• There is an interview process, which is face to face, to gather information.
• Application to be completed by the Director to make sure the particpant is a suitable candidate.
• a Medication list is requested
• Applicant's medical information will be sent to their primary care physician to make sure that the particpant.
• Once all pertinent information is reviewed, a decision can be made for enrollment.
